For Patients

The CARE Clinic will help you transition to home and assist you with follow-up care. CARE Clinic Team Members include a Nurse Practitioner, Pharmacists, Nurses, Dietitian, Medical Assistant, Behavioral Health and Social Workers. The CARE Clinic is not a substitute for a primary care doctor or specialist.

The goals of the FMH CARE Clinic are to:
  1. Make sure you are seen within one week of discharge if your providers are unavailable or if you do not have a provider
  2. Address any problems quickly after discharge
Our services include:
  1. Helping you understand your treatment plan and your role in self-care
  2. Assisting you in making follow-up medical appointments
  3. Helping you obtain healthcare equipment, such as blood glucose meters, scales, walkers, etc
  4. Removing financial barriers to accessing healthcare
  5. Providing health education on matters such as
  6. Understanding your medications and how to take them
  7. Addressing any health concerns
  8. Ensuring you have adequate support at home

Please remember to bring:

  1. All of your medications from home so we can review them and make certain that the prescriptions you have are correct
  2. Any recent discharge paperwork
