What if I change my mind about who I want to be my healthcare agent or about the kind of treatment I want?
You have the right to change your mind at any time. Please review your Advance Directives periodically to make sure the information reflects your preferences. The best way to change your Advance Directives is to create new directives. The new Advance Directives will replace the old version, but you must inform all people (family, friend, healthcare agent, physician, hospital) who have copies of your old Advance Directives that you have new ones. Collect and destroy all copies of the old versions of your healthcare instructions and/or appointment of a healthcare agent.
