Please bring the completed patient packet with you the night of your sleep study - If you have questions about how to complete a part of the packet, please call the sleep center at 240-566-4827 or you may ask your technologist the night of your study. Packets may be filled out electronically and then printed, or you may print out a blank copy and fill in by hand.
