Pre-registration is performed by our friendly staff in the Pre Access Department of Frederick Regional Health System. To assure your service is provided in a timely manner prior to your date of service, you have the ability to complete pre-registration through the power of the internet. This Web Site allows you to complete the pre-registration process from your home and other locations with Internet access. In addition, our Pre-Access staff is available Monday through Friday 7:30a-5:30p to assist you with pre-registration. You are important to us, and we are here to help you answer any questions by calling 240-566-3400 or our toll free number 800-995-1438.
This will expedite the registration process in order to decrease patient wait time on the day of your scheduled service. For unscheduled services, pre registering is still recommended and your registration will be finalized when you arrive for services. We know your time is valuable!
When to pre-register?
Please complete the pre-registration process as soon as possible after you have scheduled your appointment, but no later than two business days prior to your visit. This will allow the staff adequate time to verify and pre-certify your insurance (if applicable). If you are unable to meet this timeframe, pre-registration can be completed over the telephone the day prior to your outpatient service(s) by calling the Pre Access Department at 240-566-3400.
How to pre-register?
Pre-registration can be completed several different ways for the patient's convenience. Choose one of the following options:

Please be prepared to provide the following
Date of test or procedure
Demographic information (including LEGAL name and date of birth)
Insurance information (available on your insurance card)
First and Last name of your physician
Hours for pre-registration
The Pre Access staff is available from 7:30a-5:30p Monday through Friday.
Is on-line pre-registration secure?
The Pre Access Department supports the Frederick Regional Health System values by respecting the right to privacy and confidentiality of all patients. The information you submit will be transmitted via secure 128-bit encryption.
Will it be necessary for the patient to bring their insurance card and a picture ID?
Yes. FMH must protect information that is entrusted to us by respecting the right to privacy and confidentiality of all patients. A picture ID is required for positive patient identification and for the safety and security of each patient. In addition, your card will be scanned for future reference into you account.
Will the patient be required to stop by at registration?
Yes. Upon arrival Patient Access will obtain all necessary consents and scan your insurance cards. Also, if you are a Medicare patient, we will need to complete the Medicare Secondary Payor Questionnaire (MSP).
What is the Medicare Secondary Payor Questionnaire (MSP) and why does it need to be completed for each registration?
Federal Law requires that providers of medical services to Medicare beneficiaries determine whether or not there is other insurance coverage for the beneficiary that should be billed before a bill is submitted to Medicare. Providers, such as hospitals, must be able to document that they have screened for other insurance coverage by asking the beneficiary questions which will identify if other insurance is available. The Medicare Questionnaire contains all of the necessary questions that need to be asked of the beneficiary to determine if there is other insurance that is primary to Medicare. Accurately completing the Medicare Secondary Payor Questionnaire ensures expedient payments to providers, as well as compliance with Medicare rules and regulations.
Where should I report on the date of my service?
Depending on which FMH facility you are scheduled for, upon arrival you will be directed to the registration area where our registration staff will take good care of you. To assure our patients are on time for their service, patients may be serviced according to appointment time.
How is a wheelchair obtained when the patient arrives?
Wheelchairs are located at each facility for your convenience.
