Health Information Exchanges


FMH Health Information Exchange


We participate in the regional FMH Health Information Exchange (FMH HIE) operated by Frederick Memorial Hospital. A Health Information Exchange, or HIE, is a way of sharing your health information among participating doctors' offices, hospitals, labs, radiology centers and other health care providers through secure, electronic means.

We share this information so each member of your health care team can have the benefit of the most recent information available from your other providers when taking care of you. For example, doctors can review recent lab or radiology results whether the test was ordered by your primary care provider, specialist, a hospital, or participating lab.

Benefits of Health Information Exchange
Your health information is available to your doctors when and where they need it. HIEs help avoid repeat tests, since your providers can review tests ordered by other doctors or specialists. This saves time, reduces paper and eliminates the need to search for charts, make phone calls, or request faxes. Easy availability of this clinical data can also prevent duplicative testing, reducing expenses.

Privacy and Security of Health Data
The privacy and security of your health information are important to us. The FMH HIE follows all state and federal laws (including HIPAA) to protect patient information. To protect your privacy, only authorized users, such as your providers and their staff, have access to your health information.

What if I Don't Want to Participate in the FMH HIE?
If you opt out of participation in the FMH HIE, doctors and nurses will not be able to search for your health information through the HIE. Your physician or other treating providers will still be able to receive your lab results, radiology reports, and other data sent directly to them that they may have previously received by fax or mail. If you choose not to have your health information shared in this method, you can opt out by completing a written opt out form, included on the back of this fact sheet. We will comply with your opt out request to the extent required by applicable law. Opting out of the FMH HIE will not preclude any participating organization that already has received or accessed your information from retaining such information If you opt out, you can choose to resume participation by submitting a written request to one of our offices.

Statewide Data Exchange 
 We also participate in the Chesapeake Regional Information System for our Patients (CRISP), a non-profit health information exchange approved (but not operated) by the state. If we participate with CRISP, your health information will be shared with this exchange in ways similar to those described above regarding the FMH HIE Exchange. You may opt out and prevent searching of your health information available through CRISP by calling 1-877-952-7477 or completing and submitting an opt out form to CRISP by mail, fax or through their website at crisphealth.org. We are not CRISP and cannot submit your form should you decide to opt out.
